How can someone with spina bifida lose weight all over?

I'm 37 (38 next month). I'm living with spina bifida and been a wheelchair user most of my life. Do not take care of me when I was younger, I ate what I wanted, whenever I wanted, I wanted at any time. Now he has finally caught up with me and I can not lose weight for the life of me. Does anyone have personal experience with this who could tell me what to do? I am not able to carry anything below my waist.
